How to Apply for Makati Medical Center's Green Card?Did you know that Makati Medical Center (MMC) has a social services program that allows non-Makati residents to opt for free medical consultation and discounted medical procedures? Yep, they call it MMC's Health Service Program, more popularly known as "green card."
So how do you exactly apply for this coveted green card? The first thing you need to do is visit the Outpatient Department (OPD) located in Tower 3 of Makati Medical Center. It is at the 2nd floor of the building located behind Tower 1. You'll know you're in the right place when you see Family Mart and Starbucks just beside the main entrance of the building.
Proceed to the Social Services section. Don't be surprised to find that there are people already lined up for their application. The guards in OPD are very accommodating and they make sure that there's no "singitan" going on. Make sure that you're early so you can be first in line. Some people arrive as early as 6AM. You'll notice that in the picture above, it is only 20 minutes before 7AM and there are about 10 people already lined up in front of the Social Service window.
The window opens at about 7:45AM and all the people lined up will be asked one-by-one to sign the registration sheet. The schedules for the issuance of green card requirements are 8AM to 11AM and 1PM to 4PM. In the afternoon, registration starts at 12:45PM. Usually those who register late in the morning are entertained in the afternoon, if time permits. But with so many people trying to apply for the Health Service Program of Makati Medical Center, there are incidents when people go back the next day since they were not able to finish their application previously. So make sure that you're early.
Your initial interview will be very simple. The social worker will ask basic questions about you. From here, they will decide which documents they would require you to submit. The requirements asked from applicants vary and they will basically depend on your answers from the initial interview. The social worker will give you a piece of paper that indicates the documents/materials you need to submit.
Here's the list of requirements:
- 1x1 photo
- Birth Certificate
- Certificate of Marriage
- 2 Valid IDs
- Certificate of Employment
- Latest one month copy of payslip
- Barangay Certificate of Indigency
- Barangay Certificate of Residency
- Certificate of Non-HMO Coverage (for those with HMOs)
- Php35 for the Green Card payment
Again, the requirements asked from one applicant will not be entirely the same with another. Also, the social worker/s may add additional documents not indicated in the list above. You will be asked to return once you have completed all the documents you need to submit. They also follow a very strict rule: "No complete requirements, no interview." So please make sure that you have everything before coming back for the final interview.
